Add surgery reference for dog and cat fracture of distal tibia, pelvic sacrum,humerus, radius and ulna.

dog humeral shaft has been published recently. Next modules in pipeline, currently in production, are: proximal radius and ulna, diaphyseal radius and ulna, distal radius and ulna, tarsus, and distal tibia. Check out here for more details:
